Children's Choice Child Care Services, Inc. on FacebookDo you need help paying for child care? Child Care Assistance is available. If you qualify for financial assistance, your co-payment will be based on the size of your family and your family's income.
How to Apply for Child Care Assistance
Take the following documentation to the Child Care Services Bureau at 3401 Pan American Freeway NE, Albuquerque, NM 87107 (South of Comanche Exit on West Side of I-25). Their phone number is 841-4800. Applications are accepted on a walk-in basis Monday thru Friday between 8:00AM- 4:00PM.
You must have the following documentation:
q A completed and signed child care application (DOWNLOAD BY CLICKING BELOW).
q Picture ID for head of household.
q Social Security Cards for all household members.
q Birth Certificates for all children (not only those who would attend Children’s Choice).
q Verification (paycheck stub for one month) of income including child support, SSI.
q Verification of school attendance, if applicable (class schedule).
q Verification of residency (lease agreement, utility bill).
q The name of child care provider is Children’s Choice at ___________ Elementary School.
