Purple Cow Submission Form

Recognize a staff member who has created a unique and memorable experience for children at Children's Choice. The Curriculum Support Circle will review submissions and select one staff member to honor as “Purple Cow” worthy. The chosen staff member will sign the Purple Cow and receive a shirt, a pin, and a gift certificate for Children's Choice merchandise.
